Summary:
EmpRes Operated by Evergreen is currently looking for a Care Coordinator to oversee the scheduling, organizing and managing all aspects of a patient's healthcare maintenance and treatment for all EmpRes facilities in Oregon.
Essential Functions:
Works with center teams in all Oregon facilities to manage the Medicare/Skilled Care program Coordinates with center staff to ensure technical and clinical criteria for Medicare, Medicare Advantage, and other skilled payers are met Reviews treatment plans for all skilled care residents on a regular basis to ensure skilled criteria is met Must be expert in RAI, MDS, and PDPM payment methodology Orientation and on-going training of center IDT in areas of MDS, Medicare requirements, Managed Care requirements Works with center team to ensure appropriate reporting of services provided that impact reimbursement for Medicaid and Skilled residents Possesses leadership qualities needed to guide staff towards a common goal for the betterment of a patient's health and quality of life Maintains patient confidentiality Manages time and routine independently, and able to travel to all centers Education, Licensure, and
Experience:
Required:
RN Licensure in the state of hire Strong organizational and time-management skills Superb communication Demonstrated knowledge of care coordinating, particularly as it relates to geriatric residents, long-term care, and rehabilitation.
